Health Agreement
DOWN DOG LODGE - HEALTH AGREEMENT
I hereby certify that my dog(s) are in good health and have not been ill with any communicable diseases within the last 30 days.
AGGRESSION: I further certify that my dog(s) have not harmed or shown aggressive or threatening behavior toward any person or any other dog unless specified to staff.
TRANSPORTATION: I agree to have my dog travel in a car when being transported to and from home, on their way to Down Dog Lodge (DDL) or to a veterinarian.
FLEAS: I agree to keep my dog free of fleas by using flea prevention products such as Frontline, Advantage etc. as recommended by my veterinarian. Should DDL find fleas on my dog(s) I understand that the situation must be remedied immediately. DDL will make a reasonable attempt to contact me or another authorized contact to inform us of the situation. I understand that I will need to pick up my dog immediately and remedy the flea problem before my dog can return to DDL. If I or another authorized contact cannot be reached I authorize DDL to purchase and apply flea treatment, which I will pay for according to their current rates.
VACCINATIONS: I certify that my dog(s) have received all necessary vaccinations and have given a copy of vaccination certificate to DDL. I agree to keep all vaccinations current. If my dog(s) vaccinations are not current, I realize that I must remedy the situation immediately before my dog can return to DDL facility. A full list of required vaccinations can be found on DDL's website.
ILLNESS AND INJURIES: I understand that in a multiple dog environment, injuries and illnesses can and do occur. I agree to hold DDL harmless in the event that my dog contracts any illness, such as kennel cough, canine influenza, giardia, pneumonia, eye infections, worms, or any other contagious condition while being in its environment. I also agree to hold DDL harmless in the event that my dog incurs scratches, bites, ligament tears, or other injuries. Additionally, I agree that DDL is not responsible for any costs associated with medical care.
In the event of a health emergency, I authorize DDL to transport my dog to and from my veterinarian or a veterinarian of its choice. I agree to reimburse DDL for any and all expenses incurred in relation to caring for my dog in an emergency situation. I have read, understand, and agree to all of the above statements.
Service Contract
DOWN DOG LODGE - SERVICE CONTRACT
1. I, the undersigned, have asked Down (DDL) to provide care services for my dog on an ongoing basis. I understand that, depending upon the arrangements I make with DDL such care may include: (a) providing sanitary and safe quarters for my dog at DDL’s facility on a day care or overnight basis; (b) exercising and playing with my dog; and/or (c) properly and regularly feeding my dog. I understand that DDL and its employees will exercise due and reasonable care in all matters pertaining to my dog. In consideration of these services, and as an express condition thereof, I hereby expressly waive and relinquish any and all claims, which I may have against DDL relating to the care of my dog, with the exception of claims arising from documented negligence or willful misconduct of DDL or its employees.
2. I understand that the fee per visit to DDL facility, as well as the fee for any transportation, home care, or other services, will be agreed upon prior to commencement of such services, and will be in compliance with the current prices for such services published on a separate price sheet. I agree to immediately pay for all services rendered. I agree that if I fail to pay for DDL services as agreed, and DDL is required to initiate a court action against me, the prevailing party in such action will be entitled to recover attorneys’ fees.
3. I understand the daycare hours at DDL and that I must pick up my dog before closing time for that day unless prior arrangements have been made. I agree to pay a late pickup fee of current rate for each hour my dog remains at DDL past posted pickup time. If not, I will make arrangements for my dog to board overnight for an additional boarding fee.
4. I certify that my dog has not harmed or shown aggressive or threatening behavior toward any person or any other dog unless specified. I further certify that my dog is allowed to play with other dogs. I understand that if my dog exhibits aggressive behavior, or injures another dog or person, I am responsible for the costs of any and all medical treatment associated with any injury caused by my dog. I further understand that DDL will use any means available to control a dangerously aggressive dog, and I hereby expressly waive and relinquish any and all claims, which I may have against DDL or its employees relating to actions taken to control my dog under such circumstances. I agree to allow DDL to separate my dog from other dogs or to place a muzzle on him/her for safety purposes.
5. I understand that for my dog’s safety, and the safety of others, my dog must be accompanied by an adult and kept on a leash to and from my car when being dropped off or picked up at DDL facility, outside the building and in the DDL facility until handed off to or from a DDL employee.
6. I will ensure that my dog is wearing pet legible I.D. tags each and every day upon arriving at DDL. I understand that I am responsible for keeping my pet’s I.D. tags up to date with dog’s name and phone number. I agree to pay DDL for a temporary ID tag to wear during his/her stay at DDL, if my dog arrives without an ID tag.
7. In the event of local area emergencies, DDL is entrusted to use its best judgment in caring for my dog. I hereby expressly waive and relinquish any and all claims, which I may have against DDL or its employees relating to decisions made in this regard.
8. By my signature below, I authorize DDL to obtain necessary emergency veterinary care for my dog. I authorize DDL to engage the services of a veterinarian of its choice. If I cannot be reached in a timely manner, I authorize DDL to approve veterinary treatment (excluding euthanasia) as recommended by a veterinarian. I hereby expressly waive and relinquish any and all claims, which I may have against DDL or its employees relating to such treatment, including claims related to transportation of my dog to a veterinarian or to another agreed upon location. I agree to reimburse DDL for all expenses incurred in connection with such treatment, including food, medication and other supplies, and I agree to pay an additional fee to DDL for attending to such matters. I understand that just like in a human kindergarten, dogs in a multiple dog environment can and do get injured and contract communicable diseases. I agree to not hold DDL responsible for these injuries and illnesses.
9. I understand that DDL is not responsible for any lost or broken items such as leashes, collars, purses, food containers, toys, blankets, or any other personal items, and I leave any such items at DDL at my own risk.
I have read, understand, and agree to all the above.
Kennel Cough Disclaimer
DOWN DOG LODGE - KENNEL COUGH DISCLAIMER
Kennel Cough is common in doggie daycares. If there are ever cases in our facility, we will post a sign on the front desk to inform clients of the reported cases.
Kennel Cough is a usually mild, though highly contagious upper respiratory infection. Kennel Cough can be spread at dog parks, vet offices, doggie daycares and even meeting other dogs on walks.
Although most dogs have been vaccinated for Bordetella, they are still susceptible to the generic form of Kennel Cough, especially in puppies or dogs with compromised immune systems. Dogs may not exhibit symptoms for 5-7 days after exposure whereas other dogs can be infected and show no symptoms.
The good news is that Kennel Cough is rarely serious and most dogs recover in a few days with nothing more than coughing, slight fatigue, or decreased appetite - just like a human catching a cold. Some dogs may need to be treated with antibiotics for secondary infections. Infected dogs must be isolated from other dogs to minimize spreading. We do screen every dog for signs of illness and ensure that all vaccinations are current, but despite every precaution, it is inevitable that there will be cases of Kennel Cough at DDL.
We sincerely apologize if your dog becomes ill following a stay with us, but we cannot be responsible for the transmissions of these highly contagious diseases, any more than a kindergarten can be responsible for a cold passing between children. Unfortunately, this is one of the risks that go along with doggie daycare. If you believe that your dog may have contracted Kennel Cough, please inform our front desk as soon as possible. Thank you for your understanding!

Inflicted Damage Agreement
DOWN DOG LODGE - INFLICTED DAMAGE AGREEMENT
We understand that things don’t always go as planned. In the situation that a dogfight occurs, we do our best to determine the details and hold the correct parties responsible.
I agree to pay in full for all services needed if any of the following apply:
1. My dog instigates a fight by snapping, lunging, or attacking another dog; I will be responsible for all veterinary costs that were incurred due to injuries caused to my dog(s) and any other dogs involved.
2. All follow-up veterinary charges that are required for the dogs involved.
Emergency Medical Attention Agreement
DOWN DOG LODGE - EMERGENCY MEDICAL ATTENTION AGREEMENT
In the event that my dog becomes ill or otherwise needs immediate medical attention while at Down Dog Lodge, staff will ALWAYS attempt to contact the owner by phone and email to relay this information and determine next steps.
Should those attempts be unsuccessful and Down Dog Lodge is unable to get in contact with me, I authorize DDL to pursue emergency medical attention at my expense.
Fixing Policy
DOWN DOG LODGE - FIXING POLICY
While our Puppy Program accepts intact dogs between three and six months, we require all dogs over the age of six months to be fixed.
Titer Tests
DOWN DOG LODGE - TITER TESTS
We do not accept Titer Tests in lieu of updated vaccinations. As Titer Tests are designed to measure active vaccine antibodies in the system, it does not correlate to any specific duration of extended protection. Since every dog is different, a positive Titer today could result in a negative one tomorrow. The only way to truly understand the current standing is to test daily/weekly which is not logical or financially feasible.
Probationary Period Agreement
DOWN DOG LODGE - PROBATIONARY PERIOD AGREEMENT
After passing the two-phased Temperament Test, each dog will enter an indefinite probationary period where DDL will closely monitor their activity to confirm the mutual fit and safety for all dogs in the facility. Even upon passing a Temperament Test, if DDL concludes that there cannot exist a safe relationship with the facility, other arrangements may need to be made.
Boarding Agreement
DOWN DOG LODGE - BOARDING AGREEMENT
It is highly recommended that prior to overnight boarding that your dog attends multiple days of daycare, if possible.
Being able to comfortably assimilate into the environment is majorly contingent on the ability to understand the surroundings, which takes time. This thorough intro to our facility will help with lowering stress and separation anxiety levels sometimes associated with overnight stays.
Future Temperament Test Involvement
DOWN DOG LODGE - FUTURE TEMPERAMENT TEST INVOLVEMENT
Upon completion of a Temperament Test and admittance to DDL, please note that your dog may be used in future Temperament Tests for prospective clients.
Six Month Inactivity Reevaluation
DOWN DOG LODGE - SIX MONTH INACTIVITY REEVALUATION
Just as human personalities change over time, so do dogs!
After six months of inactivity at DDL, we require a retest to reconfirm the mutual fit in our open cage environment.
